IN-HOME ASSESSMENTS
Evaluation of the senior’s functional, social, and mental health.
PLACEMENT & RELOCATION ASSISTANCE
Evaluation and assistance with the selection of an appropriate care facility, if needed. Help in relocating the senior to the selected facility, and overseeing a smooth transition, which can include arranging pack-up services, monitoring a physical move, coordinating the sale of a home, set-up at the new facility, and facilitating the sale of furniture and items no longer wanted .
MEDICAL MANAGEMENT
Attending doctor visits, as well as facilitating communication between the doctor, client and family.
EDUCATION & SUPPORT
Ongoing communication to update the family, and providing support for both the senior and family.
INFORMATION & REFERRALS
Ongoing assistance in connecting with the appropriate community resources.
SERVICE COORDINATION
Arranging all services and communicating with all providers to ensure comprehensive care for the senior.
MONITORING OF SERVICES
Ongoing evaluation of services, as well as communication with the client’s providers to help to ensure that the senior’s needs are being met.
ASSISTANCE WITH LONG DISTANCE CAREGIVING
Acting as the eyes and ears for families who live long distances from the senior, or for those families who live in-town but have to travel frequently for work.
PERSONALIZED CARE PLANNING
Development of a comprehensive Care Plan identifying the senior’s needs, including recommendations to meet these needs.
ADVOCACY
Providing strong and effective advocacy for the senior in all aspects of his/her life, particularly with respect to medical issues, in order to ensure that the senior’s needs are appropriately addressed.
FINANCIAL MANAGEMENT ASSISTANCE
Assistance with insurance filing and medical claims, arranging for and overseeing bill paying, and helping to obtain benefits from government agencies such as the Veterans Administration and Social Security Administration.
TRANSITIONAL ASSESSMENT
The “Transitional Assessment” service may include visiting with the senior 4-6 weeks after their move to a facility (Independent Living, Assisted Living, Memory Care, etc.) in order to evaluate their acclimation to their new living situation, particularly focusing on signs of depression, anxiety and isolation.
In another example, the Transitional Assessment service may be used to provide an objective professional evaluation as to whether a senior who is already residing in a facility needs to transition to another level within the facility. For instance, an evaluation could help to determine whether it is time for a senior in Independent Living to transition to Assisted Living.
